21/35ACCEPTANCE OF EXTENDED FUNDING FOR ASSIST HOSPITAL DISCHARGE SCHEMEACCEPTANCE OF EXTENDED FUNDING FOR ASSIST HOSPITAL DISCHARGE SCHEME

The Head of Housing provided presented the report for the Acceptance of External Funding for the ASSIST Discharge Scheme.
The Portfolio Holder for Corporate was advised that Mansfield District Council had been working in collaboration with Adult Social Care to assist in the hospital discharge process at the Kings Mill Hospital. Following an audit completed by the Clinical Commissioning Group (CCG) to evaluate if the scheme was still fit for purpose the decision was made to extend the scheme for a further 2 years commencing April 2021 until 2021. The funding will total £300K over a 2 year period this included £240K from the Sherwood Forest Trust and £60K from NHS Nottingham and Nottinghamshire CCG.
The Head of Housing informed the Portfolio Holder for Corporate and Finance of the number of ways that the funding has helped to support individuals to be independent at home. Some of the ways that support was given included having a dedicated handy person service who was able to fit key safes and carry out minor adaptations, the offer of lifelines which provide 24 hour monitor and support to help residents to be able to remain in their homes for longer. Furniture can be moved within the home to accommodate hospital beds and also to fast track repairs and adapted accommodation as well as a range of advice and access to support services. The scheme has operated throughout the pandemic and was also extended to incorporate the delivery of prescriptions to help the hospital to be able to speed up discharge and this has helped to release the pressure on the wards caused by Covid. The Officer advised that there would also be the need to employ a handy person for the extended scheme as part of the funding if approved.
The Portfolio Holder for Corporate and Finance was pleased to hear that the scheme had been extended again as it was very important to maintain and build strong links with our communities and hospital.
The Portfolio Holder stated that he was happy to formally accept the Officer recommendations:
(i) That the Council accepts £150,000 per annum for the next 2 years. This will be contributions from Sherwood Forest Hospital Trust of £120,000 per annum and £30,000 per annum from NHS Nottingham and Nottinghamshire CCG.

(ii) That the 2021/2023 General Fund revenue budget be amended accordingly
